Sign the Waiver

Any person who wishes to shoot, observe, or participate in any activity at Centerville Small Arms Association (the "Range") must complete this form and accept the following agreements as a condition of, and as consideration for, their participation at the Range. All information supplied will be kept strictly confidential, and is for the sole use by the Range to identify the participant and evidence such participant's (or such participant's guardian's) agreement to hold the Range and its employees, owners, and property owners harmless for any incident or injury that may occur at the Range.

In consideration of being permitted to enter the Range for any purpose, and/or being permitted to shoot and/or observe any activity in the Range, I, [Participant's name], for myself, my heirs, successors, and assigns, hereby:

• Release, waive, forever discharge, and covenant not to sue the Range, its landlord and other property and land owners, and each of their respective owners, officers, agents, employees, and servants (collectively, the "Releasees") from all liability to myself for all loss and/or damage and any claim or demands therefor on account of any injury, damage, or loss to person or property, whether caused by the negligence of the Range, its owners, and/or the Releasees or otherwise, while I am on or upon the premises comprising the Range.
• Agree to indemnify, save, and hold harmless the Releasees and each of them, from any loss, liability, damage, or cost that such Releasees may incur due to my presence and/or participation at the Range and on or upon the premises comprising the Range, whether caused by the Releasees or otherwise.
• Acknowledge and agree that the activities of the Range are very dangerous and involve the risk of serious injury and/or death and/or property damage, and that I knowingly and voluntarily assume full responsibility for any risk of bodily injury, death, or property damage while on or upon the premises comprising the Range.
